If someone has no relatives and is in ill health, old and not able to care for herself,?

how is power of attorney dealt with. She is 91, frail and mild dementia. Would agree to anything if asked nicely which would be a problem in the wrong scenario. Cannot make decisions on any level. She has a solicitor as she made a will 30 years ago. Would that solicitor be able to make decisions for her?
